IEP Assessment Plan

What is it?

When the need for a student to be assessed arises, assessments cannot be conducted unless an assessment plan is created. This assessment plan must then be approved by the parent/guardian.

An assessment plan will contain all assessments that are to be administered to the student. Failure to obtain consent for an assessment via the assessment plan will void the assessment and restart the process for the given assessment.

Timelines

When parent/guardian gives written consent to the Assessment Plan, the IEP team has no more than 60 days to administer assessments and collect all data.

The 2 images below show a student that is to be assessed for an initial diagnosis. The student was previously diagnosed by a local agency that was taken into consideration by the school district.
